How We Work
We are happy to discuss whether care management is the right choice for you. Please contact us for a free telephone consultation.
 
We begin with a comprehensive assessment of your loved one’s abilities and needs. This assessment includes medication review, environmental evaluation, dementia screening, and review of financial, legal and medical conditions. You will receive a report of our findings along with an extensive care plan which includes recommendations and referrals tailored to meet your loved one’s specific needs.
 
After the assessment is completed, we will assist in implementing the plan of care and can further monitor the services or living arrangements to ensure your loved one’s needs are being met in the most respectful and considerate manner.
 
We can also help with crisis intervention or emergencies, paying bills, doctors’ appointments, setting up caregivers, running errands, and anything else your loved one requires. If you can’t be there, let us do it for you. You choose how we can aid you in providing the best "care at a distance" possible. We can serve as the “Ultimate Advocate” by periodically reviewing care needs and adjusting to meet your loved one’s ever changing needs, and by ensuring the care promised is the care received.
 
We do not require a retainer, and our fee-based service is a flat hourly rate, which we bill monthly. You pay only for the time that is used. We strive to be the most objective of providers, and do not accept compensation from any of our referrals.
